About Community Living Care, INC.

It is our mission to provide services to persons with disabilities in community settings. Community Living Care, Inc.(CLC) is committed to providing the highest quality service designed to address the particular needs of the individual while promoting and supporting each person to maximize their own potential. CLC will provide the individual the support necessary to establish and continually expand a lifestyle that promotes individual growth, independence, autonomy, and community integration.

Through innovation by design, our services will continue to meet the needs of the surrounding community. Our team of program workers, program specialists, and managers promote a culture of empowerment through strength-based self-fulfillment.

What are our services?

Community Living Care, Inc.(CLC) provides a full range of community based services for people with disabilities and special needs. Depending on the need of you or your loved one, we offer home and community habilitation services through the following programs: Residential, In-Home Supports, Adult Training Facility, OBRA, Senior Adult Daily Living Center, Behavioral Supports, and Rep Payeeship.

How to access our services?

Our services may be accessed through various venues. These include direct referrals through our web page at www.communitylivingcare.org, base service units, care managers, or other entities. Many of our services are funded under the Home and Community Based Waiver Program. Admissions, the provisions of services and referrals shall be made without regard to race, color, religious creed, disability, ancestry, national origin, age, sex, or Limited English Proficiency (LEP).

OBRA Waiver

Community Living Care, Inc. is now enrolled as a provider under the OBRA Waiver through the Office of Social Programs.

To receive these services the following functional eligibility requirements are necessary:

Being developmentally disabled.

Disability manifests itself before the age of 22.

Disability is likely to continue indefinitely; results in substantial functional limitations in three or more major life activities.

Nursing facility resident determined to be inappropriately placed.

Primary diagnosis not mental retardation.

Community resident who meet ICF/ORC level of care.
